Introduction

Heart-related illness has and has undermined the older people in contemporary society globally compared to the young aged. For instance, in Chittenden County, Vermont, heart diseases have been declared a Public Health Issue, about 24, 686 people who are 65 years and older are at high risk of being infected heart-related diseases. Having realized that most of them can not pay for their medical bill relying on Government sponsorship prevention strategy is critical. Consequently, awareness will be fundamental to not only the elderly but also the whole community in general on how to prevent and deal with heart diseases.

Description of Proposed Program

The pilot program aims at targeting people aged over 65 years old, since the population is believed to be at risk of developing heart disease in Chittenden County, Vermont. The program aims at proposing for the community to engage in physical activities that will reduce the risk of people over 65 years of developing heart diseases. Additionally, the program aims at making sure that individuals adopt a healthier lifestyle (Esselstyn, 2008). The program will ensure that there are health promotion efforts that will engage the at-risk population to go for screening for heart disease. Besides, the program will have specific deliverables for the society on matters of them engaging prevention activities.

With a population of 24, 686 , mostly the aged 65 years and over and at-risk of developing heart diseases. The program aims at targeting the individuals to make sure they are aware of the risks they face so that they engage in activities the program will recommend that will reduce their chances of developing the condition (Centers for Disease Control, 2019). The main goals of the program are as follows

create awareness for the heart diseases among the at-risk population
screen individuals that are at risk
strive to prevent risk factors in the population
treatment of the risk factors among the people aged 65 years and older,
early identification of symptoms for heart diseases, managing the condition among the population, and prevention of recurrent cardiovascular ailments in patients that already have cardiovascular ailments (Esselstyn, 2008). Even though the program is a prevention program, it is expected that some of the individuals piloted in the program will be found to be having cardiovascular diseases, and that is why there are efforts to deal with cases that are positive for any form of cardiovascular disease.

The small program will target a sample of 30% (7406) of the target population in Chittenden County, Vermont. The sample will be used so that the findings are generated to the whole population. This will be a pilot study of the sample so that the awareness program also integrates a health promotion project that will be used to find out the resources that can be used to target the whole 24, 686 people.

It is expected that the program will have five doctor, 25 nurses, and 23 community healthcare workers that will assist in the program. After that, the at-risk population will be offered a vision for the future in form of a summary of the favorable circumstances for the at-risk population.

The volunteers will be educated on the Healthy People 2010 goals that aim at reducing the prevalence and incidence of heart diseases among people aged 65 years and older (Centers for Disease Control, 2019). The program will employ the array of approaches that the Healthy People 2020 aims at achieving through reducing the prevalence of heart disease among the general population. However, the present program will target people aged 65 years and older.

Strengths of the Initiative

The core reason that will drive this initiative is associated with evaluating to address promoting heart disease awareness among people aged 65 and above in Chittenden County, Vermont. This awareness will be aimed at educating the already ill with heart diseases on how to reduce the effects of the disease and also encourage health checkups often.
